  1. Get the free form at an Anganwadi centre, Mo Seva Kendra, Common Service Centre, Block office or urban local body office, or apply online on subhadra.odisha.gov.in.
  2. Attach the documents listed above and complete Aadhaar e-KYC.
  3. Check the beneficiary list on the portal.
  4. Rs 10,000 a year is paid in two instalments of Rs 5,000.

  2. Show the card and Aadhaar at the hospital's help desk for cashless treatment.
